I understand that the information requested in this application will be used only for the purpose of determining suitability as a CASA/EDM volunteer.
I understand that my application will be rejected if I am found to have been convicted of, or have charges pending for, a felony or misdemeanor involving a sex offense, child abuse or neglect, or related acts that would pose risks to children or the CASA program’s credibility. Further, I understand that after the successful completion of my training, I will be expected to serve a minimum of 18 months in the CASA/EDM Program. I am willing to devote, on average, twelve to fifteen (12-15) hours each month for case duration. Finally, I have a degree of flexibility in my schedule including the capacity to devote time during traditional business hours to my case. I also agree to regularly visit the children, to whom I am assigned, where they live. I am aware of the sensitive and confidential nature of the official documents, reports and other material I will examine in my capacity as a CASA/EDM volunteer. I will discuss these matters only with those person's directly involved in the case or who will be consulted for their professional knowledge and expertise, upon authorization by the CASA/EDM staff. I agree to program supervision and agree to meet program and system deadlines that other paid professionals are asked to meet. I also understand that if for any reason it becomes apparent that my activities are contrary to the policies, goals and/or philosophy of the CASA/EDM program and their desire to provide quality services to abused and neglected children, my services as a CASA/EDM volunteer will be terminated. I submit the statements on this application are true, complete, and correct to the best of my knowledge. I understand that falsification on this application can disqualify me from consideration or can result in dismissal at a later time.
Consent to background check
I understand that under the mandate of Pennsylvania State Law, CASA of Philadelphia County conducts background checks on all CASA/EDM volunteers. I hereby consent and authorize CASA of Philadelphia County to conduct a background check on me which will include a PA criminal check, CHILDLINE child abuse clearance, and a sex and violent offender registry check. Below, I have provided my full name, date of birth, and social security number for this purpose. I understand and agree that CASA of Philadelphia County reserves the right to withdraw a volunteer from the program if, in the judgment of the organization, information developed in the course of the background check so warrants.
